Preferably at home, not requiring a Drs visit, if there is even anything they can do for this? I have been dealing with this for to long!

Magnesium? Isn't the problem hard fecal matter? Perhaps a diet change, too.
Try putting a glycerin suppository up there and letting it dissolve for about 5 minutes before each BM for the next month or so. Or just get a big tub of vaseline and gloves coat your finger really good and put it up there . I've done both for excruciating hemorrhoids and fissures. No more pain for me
I am also searching for a solution for this problem. I am in a great deal of pain and the scheriproct prescribed by my GP is not working. I know the cause of mine - 4 large babies with very long pushing times during labour - last baby 10 and a half pounds!
so there is nothing I can do about that now. Mine are not bleeding, not itching, cant feel anything much, but I feel like I have been kicked in my coccyx and like I have a bowling ball up my bum! (tmi - sorry)
Looking up anal spasms they can be related to straining on the loo, but also can be neurological - those with MS get them. I have over the years often had a feeling of lack of relaxation in the area, and find it hard to relax to 'go'
Visiting a really good massage therapist or myotherapist who will listen to you and be very gentle might also help.
