PIP Review Form - no Information Booklet?

Hi everyone,

I have my PIP review form to fill out, and it has arrived without an accompanying Information Booklet explaining how to answer the questions etc. I have been claiming PIP and before that DLA for nearly 20 years and it has always arrived with a separate booklet.

Can anyone tell me whether the review forms still arrive with this kind of info booklet, or have they stopped doing this? I tried calling the DWP phone line for PIP today to ask this question and didn't get anywhere (I was waiting for someone to pick up for an hour, after they did they put me on hold as they didn't know the answer to the question and then the line cut off... It's now after office hours.)

I just got through to the DWP PIP line even though it's after office hours and was told they don't normally send information booklets out anymore for review forms, only new claims. There's quite a bit of information now online it seems. I was going to delete my post on here but not sure I can, and perhaps this will be useful for anyone to read who might be wondering about this themselves.
