Could be any of those-- nobody knows for sure. Looks like maybe some genetic predisposition gets triggered by some assault on the system: virus, chemicals, vaccine ... There are some good researchers looking for answers. Hopefully they'll find something and then a treatment.
It sucks. Hang in there.
