It comes in a text (.txt) file. Here's the beginning of my file:

Then it continues for another 970,000 lines or so.
Basically the file has the raw data, then 23andMe does a bit of processing to list known genetic risks. But you can also use various websites which will also process the data for specific purposes. http://www.ianlogan.co.uk/23andme/minor_allele_data.htm , for example, can pull out all of your rare results and put them into a list.
The process of getting your full results? You go to https://www.23andme.com/you/download/ and sign in, then you can download them. It's good to pick somewhere specific to save them to where you can easily find them again, like your desktop.
