Although from 2012, this book by Atty Jennifer Jaff (who passed in same year of Crohn's) helped me in my Social Security and group long term disability cases to fight for and eventually win my benefits without a lawyer.

I can mail this free of charge to whomever might need it in the United States. Just passing along the good will and hopefully, good luck.
