The study was about glial involvement in depression (MDD), but the interaction between astrocytes and microglia and neurons could also play a role in ME symptoms. Even small variations in those interactions could result in serious symptoms, but be difficult to find via present diagnostic techniques.

This would fit the difference in time delays for physical vs cognitive exertion triggering PEM. Physical exertion sends signals to activate glial cells with a delay (24 hrs for IFN-g, for example), while cognitive exertion could affect those glial cells directly, resulting in a shorter and less consistent delay.

While such subtle shifts in brain cell functions might not show up readily via present scanning techniques, maybe they can be found if they are intentionally looked for.

The study was just a "comprehensive literature search using widely used online repositories such as PubMed and Google Scholar", so no new information. However, looking at old information from a new perspective can still offer new insights.
