One thing... it seems to me the author is confusing "exercise intolerance" with post-exertional malaise. I'm not sure how "exercise intolerance" is defined, but the author refers to "impaired muscle function" and says,"...as we get older, there is a gradual intolerance to exercise, particularly in the frail elderly." This kind of exercise intolerance occurs during exercise and limits the amount of exertion possible, which is different from PEM, which occurs hours to days after the exercise. In 2-day exercise studies, patients with ME/CFS can often show normal exercise capacity on the first day. The abnormalities don't show up until the second test.

This doesn't mean the protocol can't be helpful, just that the rationale is based on faulty information.
