I just moved to a new medical clinic for my care, and it has an exercise physiologist on staff. I hope to talk to them at some point. The idea of a maximal exertion test is to exercise maximally - you will know when you've reached the max as you will have to stop, because you feel like you're about to pass out (severe dizziness etc). I was on the bike for almost 18-19 minutes and I wouldn't have described it as "relatively little effort at all"...
