Hi All,

Thank you for the support so far. I need to let everyone know that this is the LAST week for completing your questionnaires and I MUST have them completed and returned by Sunday or I cannot guarantee that your information will be used in the study.

Again, if there are any other volunteers that would be willing to take part in the study, please don't hesitate to contact me and supply your email address so I may sent you a copy of the questionnaire to complete.
