If anyone wants to submit a FOIA request, this is not hard to do.

You can go to Rocket Lawyer and they will prompt you with questions. You fill in the answers and they provide you with a letter for your FOIA request. It will cost you 99 cents to print out your letter from Rocket Lawyer.

NOTE: HHS employee who handles FOIA requests is Glenn Voelker (PSC/AOS), but you should make sure to send a copy to Susan R. Cornell, J.D., Freedom of Information Officer, NIH
