urgent: need help with Hyperthyroidsm, Thyroid "storm"

Hello everyone,

I am writing this in a ICU room tending to my mom who is being treated since Saturday afternoon for which the ICU doctor calls a thyroid storm.

"Thyroid storm, also referred to as thyrotoxic crisis, is an acute, life-threatening, hypermetabolic state induced by excessive release of thyroid hormones (THs) in individuals with thyrotoxicosis. Thyroid storm may be the initial presentation of thyrotoxicosis in undiagnosed children, particularly in neonates. The clinical presentation includes fever, tachycardia, hypertension, and neurological and GI abnormalities. Hypertension may be followed by congestive heart failure that is associated with hypotension and shock. Because thyroid storm is almost invariably fatal if left untreated, rapid diagnosis and aggressive treatment are critical. Fortunately, this condition is extremely rare in children."

Anyone familiar with this and/or has had it themselves? How did you develop this condition(s) and how were you diagnosed, tested, treated and whom did you see for it?

I am literally shaking and do not know what to do, where to begin as this whole incident is sudden. I also need to be strong and healthy during this whole ordeal and not be anxious. The symptoms of the hyperthyroidism from the links I've posted were already occurring long before her taking herself to the doctor on her own will because of rapid heart rate/tachycardia and GI pain. It started almost a month + with ringing in the ears/tinnitus, severe fatigue, needing to sleep often in the middle of the day, diarrhea and trouble breathing, rapid heart rate (up to 200 beats per minute). My mom did not eat much and has irregular eating patterns and does not drink much water/electrolytes, sea salt, iodine, se, thyroid support. Does not take medication and did not see the doctor earlier when everyone including me told her to. She is under heavy medication for her thyroid, low blood pressure, heart rate, antibiotics and although everything has normalized; I am worried this could be ongoing and we need to force her to do more. I can't handle this and she looks so tired and I can't think think myself. love her dearly and hate to see this happen to anyone. I am going to fall asleep in this chair and will try to rest my mind (can't for the last day and a half).

Am so sorry to hear this and will say a prayer for your mom. Is she getting good care in the ICU? Am assuming that an endocrinologist is monitoring her thyroid plus other specialists?

I have Hashimoto's (autoimmune) hypothyroid and have heard of cases of thyroid storm, Hashi's toxicosis, and Hashi's encephalopathy but these have not happen to me. Does she have Hashimoto's where her thyroid swings both hypo and hyper or is she straight hyperthyroid?

Am tagging someone who might have feedback and knows a lot about thyroid issues @Misfit Toy.

Thyroid storm is one of the best known and understood medical emergencies and in general once medical care has been started risks and complications reduce very quickly. It is usually a manifestation of autoimmune hyperthyroidism or Graves' disease. In the long term Graves' disease is usually very satisfactorily controllable with one or other of a range of treatment options. Hopefully the panic will very soon pass. If there have been complications like infection acutely it may take a bit longer to get her well but within a week I would expect someone to be stable and rapidly improving.

A thyroid storm put me in the hospital with a bad case of Atrial fibrillation. The direct cause was an over active thyroid, which was later treated with methimazole. If the hyperthyroid is caused by an immune related issue then things get a whole lot more complicated, which was the case for me.

Urgent update... My mom is not responding and is moving while sleeping(?) and reaction from medications. She had MRI done for stroke and it came back negative. But the Dr suspects something for unresponsive condition after making progress responding, talking and being able to control her own extremities. They suspect a silent seizure and will do a eeg this morning (it is 2;55 as i type this in icu). Anyone know what is going on? Doctors believe this (carotid artery? seizure) is independent of the thyroid storm. Is it from hypertension or the thyroid. He mentioned that issue with her artery/vein could be longstanding and something going on for a while. I don't know. I am so tired. imagine what my mom is going through.
Do not know what is going and if I should do something else than watch. I mean, what else does one do in this situation other than go about life. Possibly ask to be move to Stanford again. The Dr we spoke to said they have the same procedures but I get more of them to see constantly; keep up hour to hour possibly. And converge in bigger groups to figure things out. Not sure if the travel is possible. I'm trying to stay calm (not eating at all except for honey, colostrum, coconut butter or sleeping much). I'd like to allow the process to unfold but I am not comfortable not in the slightest because I have anxiety issues that stem from fear all my life. And agian, this is not normal based off what I see (medication, condition changing rapidly) and the people here who have yet to figure out what is going on.

Personally, I don't even know what to think than lament... Is this normal as this has barely been the 4th day here?

if you don't mind; how were you treated for your thyroid. My mom let these symptoms prolong for a while. And the Dr did check for Grave's disease after i asked since i think i have it too ( i have hypo and someone told me that I could have grave's disease). Thyroid diseases/autoimmune is all new to me despite I having it and treating it.

Don't know. she was doing so well and talking, wakeful and conscious on Monday and ever since Tuesday afternoon, she is not responding well to whatever is going on. The medication/drugs have not changed.
What should I be concerned about that I can inform the doctors as they make the rounds?
Specific questions/concerns?

Are there anything i can do?
Something that i should pay attention to? further problems

And is there anyone else who knows more about what is going on re; thyroid, behavior/unresponsiveness, possible reaction to medication?

Unsure what qualifies for good care. Have not been to icu for prolonged periods to witness other than now..except once. I know doctors follow the same protocol based on my conservation with the Dr here.

I do not know if she had/has hashi's. Didn't know Hashimoto's changes from hypo to hyper like that.
Is this something you've mentioned that is of relavance?

I am so tired. notthing compared to what my mom is going through.. I need to rest, sleep, meditate/pray even; was told at length by RNs here to do so but can't sleep well here in ICU. Who does? I am here alone doing shifts with my sister and brother in law as we switch whenever we feel like it. Want to stay here for as long as possible to see everything. I thought i couldn't function with whatever i am going through... anyways, i have to try to grab some sleep. tired, it is 3;23 now after finishing this.

Yes, Hashi's can change from Hyper to hypo b/c the thyroid tries to fight and releases too much hormone and then pulls back. (That is my non-technical explanation LOL). I do not know if your mom has Hashi's or if this was relevant but mentioned it just in case. Am still praying for her.

I had this happen while being on too much medication and it is awful. AWFUL.

Is she on meds? Or, is this Graves Disease? Graves is very hard to treat. I am sorry I am just getting back to this as a lot has been going on on my end with work and health.

How is she now?
