What is the general opinion of the italian doctor Umberto Tirelli? One
of my ME/CFS friends from Italy is seing him and mentioned a cost of
350 € per visit.

Yes, I went to his CFS department. They will take you seriously there and try to exclude anything that can be excluded. They're also very interested in the chronic infection aspect. They're actually a publically financed cancer hospital which became interested in CFs due to similarities to post chemo symptoms. They're more conservative and cautious than private doctors. They prescribe acetylcarnitine and some supplements, low dose cortison, immune modulators, antibiotics / antivirals, depending on what they find and how you respond. My experience has been positive even though I decided to pursue a different treatment approach in the end.
