Ill controls with another disease that is not curable, badly understood, and has as much stigma and other issues?

Also, it needs to be disabling, have no or few effective treatments, systemic manifestations, a high symptom burden and be known to affect the brain amongst other organs. Something like mitochondrial encephalomyopathies would be ideal.
