    I haven't digested all of this yet, but the bit about the PACE trial "risk of bias" seems very pertinent.

    I know that feeling very well. I often find myself thinking "I should have included X, Y, Z...."

    Somebody mentioned "fidelity to the protocol" is a useful piece of jargon to use in a case like this.

    I think you're thinking of a different Dr Whiting, in Queensland Australia (as the Whiting who did this review isn't a clinician as far as I know).
