Anyone heard of bad stomach pain around the apendix/kidney area from inosine?
Probably Uric acid. In a inosine study on MS 1/4 of the participants developed kidney stones. Ive got this symtoms sometimes even stranger I sometimes got acid taste on my skin. It could be important to take meassures to avoid kidney stones. Drink a lot of water or other fluids and you can also take potassium citrate to countereffect UEA.

There aren't many Mastocytosis/MCAS specialists around. What part of the world are you in?

Mastocytosis/MCAS is somewhat treatable. I am myself using NeuroProtek with some success. The ME/CFS is a mast cell disorder (hypothesis) thread has some info on this.

but would this guarantee other symptoms of mast cell disease like (itching), urticaria (hives), headache, gastrointestinal symptoms (including diarrhea, nausea, vomiting, abdominal pain, bloating, gastroesophageal reflux), andhypotension (low blood pressure)....
