Orthostatic intolerance is a frequent problem for those suffering with dysautonomia.

Dysautonomia, most commonly experienced as postural orthostatic tachycardia syndrome (POTS), is a recognised co-morbidity of ME/CFS. There is little debate surrounding this and much research literature exists to support such an association.

Understandably, considering the relative frequency with which some or all of the symptoms are reported, much discussion has occurred among patients, clinical experts and researchers, as to what might be causing the condition, and what role it might play in our understanding of ME/CFS. And there is a suggestion from some, that dysautonomia could be used to create a distinct subgroup beneath the ME/CFS umbrella.

However, dysautonomia is still treated with uncertainty by the medical community in general, especially as doubts remain over the distinct nature of its constituent parts, over exact causation, and with regard to the efficacy of currently prescribed methods of testing, diagnosing and treatment practices.

This leaves the majority of those affected with few places to turn. Despite the few who are diagnosed and have their symptoms properly acknowledged, and even if they do receive treatment that leads to improvement, this effective lack of action from the medical community is not a satisfactory situation for the community of patients as a whole.

Dysautonomia and autonomic dysfunction

The first point to appreciate is that dysautonomia itself is not a single condition. Dysautonomia is an umbrella term used to describe various diagnoses relating to the autonomic nervous system, that have resulted in the malfunctioning we have come to recognise as autonomic dysfunction.

Many different diseases can cause this state of dysfunction and they are generally categorised as either primary or secondary. POTS falls under the primary dysautonomia heading, alongside other diagnoses such as neurally mediated hypotension, inappropriate sinus tachycardia and pure autonomic failure, with the two former conditions also being seen alongside POTS and ME/CFS.

Rather than separate, strictly defined conditions, these diseases are best thought of as a spectrum under the parent heading of dysautonomia, meaning that patients can either have only one of the conditions or, as is frequently seen, have several of these conditions simultaneously. Secondary causes arise as a result of other medical conditions, such as diabetes mellitus, Sjögren's syndrome or multiple system atrophy.

Given the many different possible causes for a dysautonomic state to arise, it would be logical to assume that the symptoms and severity of dysautonomia vary greatly between patients. While this is often the case, the following symptoms are most frequently found across conditions:
  • Excessive fatigue and malaise
  • Excessive thirst (polydipsia) often resulting in frequent urination (polyuria)
  • Lightheadedness, dizziness or vertigo (sensation of the room spinning)
  • Feelings of anxiety or panic
  • Rapid or slow heart rate (often alternating between the two)
  • Orthostatic hypotension, sometimes resulting in syncope (fainting)
  • Gastroparesis sometimes resulting in nausea, vomiting and IBS
Neurally Mediated Hypotension


The Head up Tilt-table test is a vital component in making a successful diagnosis of dysautonomia. The linked video (click picture) explains how the test is done and looks at other tests for dysautonomia.

Neurally mediated hypotension (NMH), as with most diseases relating to a dysfunction of the autonomic nervous system, is not yet fully understood. The exact cause remains a mystery and yet it is recognised as a miscommunication between the brain and the heart following a stressor event, such as a change in posture, moving from a warm to cool environment or sometimes emotional stress.

This specific situation often occurs when the autonomic nervous system is required to respond quickly, whether that be to stimulate vasodilation upon entering a warm room, or with a release of adrenaline (the 'fight or flight' response), following a stressful event.

Owing to the dysfunction, the response to these events is abnormal, and causes reduced blood volume within the ventricle of the heart leading to a rapid decrease in blood pressure. Known as a vasovagal response, it is mediated by the autonomic nervous system, and most specifically the vagus nerve. The effects of this can range in severity, from causing mild symptoms such as momentary lightheadedness and fatigue, to causing the patient to faint almost immediately.

While this condition sounds quite serious it is very common within women of child-bearing age and often goes undiagnosed due to the relatively mild symptoms that most patients experience. Nonetheless, NMH can also be quite debilitating when in its more severe forms.

Several studies have reported a higher incidence of NMH in ME/CFS patients and yet the condition often goes undiagnosed. The problem is perhaps exacerbated because the symptoms can be confused with the primary diagnosis and because no specific testing is advised.

NMH can be treated with a combination of an increased salt and water intake, alongside drugs that help regulate blood pressure. Some drugs also function by allowing the kidneys to better retain sodium, while others inhibit the body's response to adrenaline. Additionally, it's very important to review any current medications or supplements with your doctor to ensure that these do not include any drugs or vitamins that have the potential worsen the situation.

Inappropriate Sinus Tachycardia

Inappropriate sinus tachycardia (IST) is defined by an individual’s resting heart rate being abnormally high, often greater than 100 beats per minute (bpm) or rapidly accelerating to over 100 bpm without an identifiable cause (although small amounts of exercise, emotional or physical stress are often triggering factors as seen with many other conditions of dysautonomia).

IST can be caused by an anatomical problem, the heart sinus have an abnormal structure or function, but it is thought to occur much more commonly as a result of dysautonomia. As with many other conditions of dysautonomia, diagnosis is often difficult to achieve and is a diagnosis of exclusion which often means excluding all other potential causes of tachycardia, measuring the heart rate over a 24-hour period and documenting a complete medical history including all current symptoms.

Treatments for IST are still in their infancy, they may include self management, pharmacological therapies and, in severe cases, invasive treatments -- it must be stressed that invasive treatments are usually only undertaken if an anatomical defect is the underlying cause. Medications may include beta-blockers, antiarrhythmic agents and calcium channel blockers among many others.

Postural Orthostatic Tachycardia Syndrome

POTS is generally defined by a heart-rate increase of at least 30 bpm when moving from a supine (lying) position, to a standing position. This change can occur anywhere from 1-10 minutes following the change of posture. Often this is tested on a head upright tilt-table test.

The normal response to a change in posture would be a narrowing of the blood vessels to maintain blood supply to the upper body and brain, to counter gravity pulling blood into the lower limbs. In POTS, the heart rate increase happens because the dysfunctional autonomic nervous system (ANS) fails to execute this change in blood vessel dilation. In attempting to compensate for this, the brain directs the heart to pump faster, and increase supply, but this is not a sufficient alternative, and the symptoms associated with POTS are witnessed.

Symptoms are also closely related to the patient's position, often becoming more exaggerated when standing and then easing when supine. Treatment is targeted at maintaining sufficient blood flow through the upper body and brain. It is often achieved by attempting to increase blood volume with an increased fluid intake, by maintaining a sufficient blood pressure through medication and increasing salt intake.

Attempts are made to improve the fitness of the patient to increase the capacity of the skeletal muscle pump. Unfortunately, this protocol is often very difficult for patients with ME/CFS, given the post-exercise malaise such exercise has the potential to cause, and needs to be handled with care.

Pharmacological treatments are very similar to those seen in NMH and IST, such as beta-blockers and blood pressure medications. However these drugs are often used to help the patient undertake a greater degree of activity, with the aim of improving the success of non-pharmacological treatments. POTS is somewhat unique in that it is often further divided into the following sub-types (many of these groups are not mutually exclusive, meaning patients may fit multiple categories):
  • Partial dysautonomic POTS: This sub-group is defined by the onset of POTS symptoms being acute following an infection, vaccination or sometimes a stressful event such as pregnancy or bereavement. Not much is known currently regarding the pathophysiology of acute onset POTS, although there is emerging evidence to support a potential autoimmune causation, with recent findings indicating the presence of antibodies targeting ganglionic receptors (AAG) within 10-25% of POTS patients. Interestingly, the prognosis for this type of POTS is better than many others. Roughly 50% of patients experience a complete recovery, or a very significant improvement, in symptoms over a 2-5 year period following the sudden onset.
  • Hyperadrenergic POTS: This is somewhat rarer than other forms and is estimated to account for 5-10% of those diagnosed. These patients experience many of the same symptoms and clinical findings as standard POTS, however alongside this there are further clinical findings and potential symptoms. Patients suffering with hyperadrenergic POTS often complain of extreme tremulousness, anxiety, cold and sweaty hands, and high blood pressure upon standing. Hyperadrenergic POTS is thought to occur due to a genetic (inherited) abnormality, that manifests as rapid increases in the concentration of noradrenaline upon standing, and which results in the aforementioned symptoms. This is often tested at the same time as a tilt-table test. Treatments are targeted at inhibiting sympathetic outflow, fundamentally blocking the binding of the excess noradrenaline and hence stopping the symptoms. One such drug is Clonidine, an alpha 2-antagonist.
  • Developmental POTS: This is thought to be a sub-group of partial dysautonomic POTS that occurs primarily in younger patients. The onset is generally around the age of 14, often following a growth spurt due to puberty, with the vast majority of affected patients being younger women. Developmental POTS varies greatly in severity. Some only experience mild orthostatic symptoms while others are severely disabled. Many patients will present with urinary and gastrointestinal problems alongside the more common symptoms of POTS, indicating the partial dysautonomic state. The majority of the patients with this type of POTS will find their symptoms improve over time, with 80% of patients experiencing a near full recovery by the time they are in their mid-20s. Treatment protocol is near identical to that used for partial dysautonomic POTS.
  • Deconditioning POTS: Studies have shown that POTS with deconditioning is similar in presentation to standard deconditioning which normally occurs due to prolonged bed rest. The combination of sensory sensitivities in these patients suggests the possibility that following an initial event or illness that triggered the onset of orthostatic symptoms, reduced physical activity leads to more complete deconditioning. There is however much debate as to whether this form of POTS is distinct from others or is simply an exacerbation of standard POTS, worsened by reduced activity and hence degradation of the skeletal muscle pump. Treatment is often the same as partial dysautonomic POTS.
  • Hypovolemic POTS: There is some debate as to whether this sub-group is indeed distinct from all the others outlined. It is defined by patients who are very sensitive to altering salt intake and can successfully adjust their plasma volume and blood pressure control with salt intake alone. The debate centres around whether this is due to a distinct pathological problem as the root causation of the condition, or simply reflects a lesser extent of dysautonomia.
Why are ME/CFS and conditions of dysautonomia seen together so frequently?

This is one of the biggest questions facing ME/CFS researchers today. As is well known, ME/CFS is currently by definition a diagnosis of exclusion. Because of this it is fairly clear that ME/CFS patients are not a single homogenous group. Under this disease heading there likely exist a vast array of conditions of which some will be purely conditions of dysautonomia, while others may only incorporate a small degree of autonomic dysfunction secondary to another condition, and similar to secondary POTS as previously discussed.


Prof. Julia Newton is renowned for her ambitious work exploring dysautonomia in chronic diseases.

One of the leading researchers investigating how autonomic dysfunction relates to ME/CFS is Professor Julia Newton, at the University of Newcastle. Much has been discussed of her work both here at Phoenix Rising and in the ME community at large. This degree of interest speaks volumes to the promising avenue of research that autonomic dysfunction presents for not only ME/CFS, but many other conditions of as yet unknown origin, such as conditions that still present symptoms despite being treated. This is often seen in a plethora of autoimmune conditions, many of which Prof. Newton has also studied, querying the role that dysautonomia may play in them.

A Sutcliffe/Newton study from 2010 states that “... orthostatic intolerance is a frequently described symptom in both NMH and CFS, with recent studies confirming lower blood pressure in CFS compared to controls.” An earlier paper, 2007, from this group further states that the “Symptoms of autonomic dysfunction in chronic fatigue syndrome” provide significant evidence that “in the Orthostatic Tolerance domain … CFS patients had mean scores [on a well established questionnaire about ANS symptoms] almost four-fold higher than controls.” In addition to this, “A particularly strong association was seen with symptoms of orthostatic intolerance, suggesting that abnormality of dynamic blood pressure regulation is particularly associated with fatigue severity in CFS/ME.”

This hypothesis of abnormal blood pressure regulation occurring as a result of a greater dysautonomia is supported by the work of David Newton, at the University of Dundee. Their study from February 2012 and an earlier one from 2011 outlined their research, and a discovery that ME patients have measurable large and small artery endothelial dysfunction, relative to age and gender matched healthy controls. This endothelial dysfunction could easily be explained by problems with the ANS, which has a controlling effect upon the endothelium.

There can be no denying then that, given the evidence, dysautonomia may be responsible for many of the symptoms of ME/CFS. And while we have yet to solve the puzzle of ME there are potential treatments for dysautonomia which could significantly help morbidity. However, the science pertaining to dysautonomia is still emerging, not unlike that of ME/CFS, and this does unfortunately put the brakes on research and clinical care. Going forward, the biggest question is going to be whether the dysautonomia we are seeing is of primary origin or the result of a secondary problem in a wider picture. This is a question only research into many avenues can truly answer. Hopefully we won't have to wait too much longer to see all the pieces of the arduous puzzle fit together.

Hi, thanks for your concerns. It's a difficult problem because the terminology around dysautonomia is not always used consistently which leads to confusion as we have here. In this instance I am simply referring to orthostatic intolerance as a worsening of symptoms upon upright posture. The quote you have makes it sound as if orthostatic intolerance itself is a disease whereas I've always seen it discussed as a consequence of conditions of dysautonomia. You could refer to conditions such as POTS and NMH as conditions of orthostatic intolerance because they often present with orthostatic intolerance but the orthostatic intolerance is simply a result of the dysautonomia.

I used the wording 'problem' in my quote simply because orthostatic intolerance isn't itself a symptom but a worsening of symptoms upon upright posture.

It's things like this that make writing about conditions of dysautonomia so difficult, I think there needs to be an overhaul of the terminology at some point.

Thanks for the clarification Legendrew. This caption jumped out at me too cause I have NMH rather than POTS and OI is a big deal for me. The caption, to me, makes it sound like OI is only found in POTS.

What about something like: "Orthostatic intolerance is a frequent problem for those suffering with Dysautonomia." ?

I have high blood pressure. Does that mean I don't have this? I'm too tired to think or read the whole thing. My blood pressure medicine reduces the sodium in my body. Not sure if that means anything.
 
I am researching gamma globulin shots and just found this quote from Dr. Jacob Teitelbaum , which seems applicable:

"Of special interest to me is research showing a relationship between the immune dysfunction, neuropathic pain, and autonomic dysfunction being found in fibromyalgia. Researchers have found that over 50% of people with fibromyalgia have what is called small fiber neuropathy (SFN). Other researchers were finding an increased prevalence of IgG1 and 3 subtype antibody deficiencies. Interestingly, both of these respond well to intravenous gamma globulin (IVIG). Although most people do not need this aggressive treatment, it has been very helpful in the 5% to 15% of the sickest people who do not respond adequately to other treatments, and especially in those who have severe autonomic dysfunction with low blood pressure and orthostatic intolerance; that is, people who are often bedridden. Dr. Mark Sivieri in Laurel, Maryland, brought this to my attention, and is doing outstanding clinical and research work in exploring the effectiveness of IVIG and the interrelations between IgG subtype deficiency, small fiber neuropathy, dysautonomia, and response to IV gamma globulin. I suspect that he will have some very interesting findings to release over the coming years."
 
Interesting article. Thanks. Especially for pointing out how often this gets missed in pwcs.

My bp and hr stay within an acceptable range so my doctors dismissed my complaints even tho my OH is well documented. I "think" they're missing out on the effects of hypoperfusion. In fact, most medical professionals I've seen think just because my hr and bp are within normal range, I couldn't possibly be feeling light headed.

Dr Peckerman wrote an article, or did an interview, on hypoperfusion. Solving this would probably make me functional. As it is, I have to give my body an hour to recover from getting potsy after only 10 minutes of standing.

I'm sure people who don't understand this think I'm "functional" just whiney or bitchy or daffy ; ) most of the time. My neuropsych testing proves otherwise. I feel so much better if I get the chance to recover from being potsy.

lotus97. I have severe Hyperadrenergic POTS (it put me into hospital recently and required me to have a drip of a couple of bags of saline). If you have POTS and orthostatic hypertension.. Im concerned about the drug your said your doctor has you on for the hypertension esp since you said the drug you have been put on lowers salt as often those with ME have more then one type OI issues.

1/ you could have hyperadrenergic POTS along with also having Hypovolemic POTS.
2/ It can be low blood volume which can be the cause of orthostatic high BP in which cause the drug you are on would make things worst (hence why I recently was put onto a saline drip when I had a resting heart rate of 130 and a BP of 180 and got dehydrated due to having POTS... peeing out my fluids too much). Dr David Bell at times gives his hyperadrenergic POTS people saline IV to best blood volume at times of severe hypertension (where he knows the person has low blood volume).

(note.. I found the orthostatic hypertension associated with it may take a little time to come down and doesnt necessarily come down right away when laying down due to the excess noradrenaline in the body takes time to clear..but if you have high BP caused by Hyperadrenergic POTS, it would greatly increase during standing or being up on feet.. (not necessarily right away thou it can happen almost right away too eg my BP can go up 50 points in 1 minute of standing.. it used to thou take being on my feet for half an hour or so for it to suddenly spike up high).

Clonidine is the drug to be on if you have Hyperadrenergic POTS as Clonidine is not only a drug used for hypertension but also a drug works by "lowering noradrenaline" (which is what causes the high BP in hyperadrenalic POTS)...and for those with ME who has both swinging BP like I do (both high and low) it can also work as a BP stabiliser drug due the way it works on the noradrenaline (so doesnt seem to affect my low BP much at all).

Those with Hyperadrenergic POTS ie with hypertension, may also need florinef on top of the Clonidine and hence also may need to increase salt (not lower it). You need a good doctor who understands this kind of dysautonomia as florinef, salt and also saline IVs are contraindicated for normal kind of high BP (non orthostatic kind).

I read on Michael J. Fox's Foundation website that POTS is caused by low norepinephrine, and that Northera just got approved to treat it. They are also testing l-tyrosine (alone, without BH4). Anyone have a comment on that?

It's pretty well known to be one cause of orthostatic intolerance, though I'd hesitate to call it POTS. I have repeatedly tested low for norepinephrine, and have found an NRI and Yohimbe (represses a norepinephrine-supressing gene which is over-expressed in ME patients after exertion) to be very helpful. But it sounds like low norepinephrine is more involved in low blood pressure (NMH?) than in POTS. Though there is some overlap, because the body will often (but not always) respond to the low blood pressure or pulse pressure with tachycardia.

So I'd be interested to know if it's supposed to help with POTS accompanied by high or normal BP, or just POTS associated with low BP.

It certainly wouldnt help if someone already had high.. I had crazily high levels.. so high that they didnt believe the results of 2-3of my noradrenaline tests (they put them down to lab errors.. but then I finally got one back which was in high range but not crazy high range and they accepted that one.

This new drug sounds a real worry if they dont test the persons noradrenaline levels and a person for orthostatic hypertension first before its used. Ive found that even some POTS specialists are not aware that the hyper kind of POTS exits (I had one POTS specialist tell me its not possible.. obviously she hadnt read any of the research on that kind of POTS!!).
